H3C, a company that provides chronic care management software, recently filed federal paperwork representing a $250k raise.  This raise is part of a continued effort by the company to expand its healthcare software products.

Based in west Omaha, H3C provides software that helps doctor’s offices provide a variety of services, including chronic care management for patients with a variety of chronic illnesses – such as diabetes or hypertension.  In addition to this CCM software, H3C has transitional care software and strong partnerships with a variety of other software companies. The company announced in early July one of these partnerships on the company’s blog ( www.h3ci.com) with Omaha based Ensocare, a Cquence healthcare company.

H3C’s management has significant past experience in building high growth companies. These managers include Michael May, who was the CEO of Sitel – a successful Omaha company founded in the 1980s, and Dave Vana, who has worked on a variety of venture backed startups, including Valid Voice, Bloom.com, and IsecureTRAC.
